Quick Apply Full-time Full Job Description The responsibilities of the Patient Care Coordinator is to ensure that patient care and services are delivered appropriately as well as the supervision of clinical personnel.
Monitors compliance with frequency and duration of visits based on established plan of care.
Assists with staffing assignments and scheduling for additional disciplines if ordered after admission.
Assists with ordering of DME requests if ordered after admission.
Monitors field staff schedules and coordinates when the respective visits are due with appropriate type of assessments.
Responsible for communicating status of patients who were hospitalized and obtaining updated orders for resumption of care.
Responsible for faxing Transfer Summary Report to patient's PCP and receiving facility where the patient was transferred to.
Responsible for faxing all Plan of Care orders to physicians and uploads to patient's chart once received with physician's signature.
Uploads clinical documents related to physician orders, lab results, progress notes including Face to Face encounter documents.
Communicates with field staff daily to coordinate schedules and provide assistance in tasking appropriate visit forms.
Works with clinical team to gather, collate, and report patient care summary information weekly and as needed.
Promptly responds to field staff's requests and concerns.
Participates in patient care conferences and in services.
Runs specific reports related to Episode Analytics, Patient Activity, LUPA, Transferred and Schedule Reports weekly and as needed.
Ensures that admitted patients receive services in accordance with established plan of care.
Attends meetings and participates on committees as required.
Performs other duties as assigned.
Minimum Qualifications of the Patient Care Coordinator include:
Registered nurse or Licensed Practical nurse or License Vocational nurse with current licensure to practice professional nursing in the State.
Graduate of National League for Nursing accredited school of nursing.
BSN degree is preferred.
Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.
Complies with accepted professional standards and practice.
Complies and maintains current CPR certification.
Complete TB test and Physical Examination within 6 months or 15 days after hire date.
Demonstrates excellent observation, verbal and written communication skills.
Minimum of two (2) years or professional nursing experience with one (1) year of supervisory experience.
Home Health (RN/LVN) experience is a plus.
