3 Santa Clara, CA Santa Clara, CA Full-time Full-time $26 an hour $26 an hour The Driver/Placement Coordinator for the Homeless Housing Problem Solving and Call Center has the unique opportunity to house homeless individuals and families by transporting them and their belongings to an emergency shelter program that has been pre-arranged.
The Driver is an important member of a team that is assessing and addressing challenges faced by a caller and providing immediate solutions that ensure their safety and get them off the streets.
The housing problem solving approach supports the effective implementation of homelessness prevention, diversion and rapid exit strategies which are part of the coordinated entry process.
If needed, the caller is also referred and transported to one of the open shelter beds available in the county.
When needed, the HPS program will have the financial resources needed to help households avoid entering the homeless.
This resource will be available to other community partners.
The Homeless Services Call Center is in full operation 7 days/week, 10 hour/day.
It is an integral part of the County's Continuation of Care and Coordinated Entry System to end homelessness.
ESSENTIAL DUTIES AND
Responsibilities:
While waiting for a BWC transport, the driver/placement coordinator duties include:
Answers daily routine and emergency phone telephone calls related to unhoused individuals seeking emergency shelter placement Completes triage form with caller collecting personal information needed for placement:
demographic information, contact information, referring agency contact, health questionnaire Provides information to callers related to homeless services in Santa Clara County, locations of shelters, hospitals, cooling center, medical facilities, outpatient clinics, etc Refers caller to other service agencies as necessary (ie.
Reentry Resource Center, Domestic Violence providers, Social Services Agency) Uses Motivational Interviewing, reality testing, empowerment, negotiations and mediations to empower callers to find own resources for housing to divert from shelters Enter all data into HMIS logging calls, services, outcomes, transportation, etc Escalates critical and time sensitive situations to BWC Management Duties as a driver include:
Responds to Calls from BWC Staff to pick up client and take to designated location.
Completes all required documentation in Binder located inside vehicle logging dates and times of travel and to what location Provides a warm handoff of individuals or families to emergency shelter program staff Assists clients in transferring their personal items in and out of vehicle Maintain vehicle per agency policy and procedure Other duties as assigned
Qualifications:
Ability to work independently, utilize data base for all data entry and demonstrate non judgment and empathy to all callers.
Bi-lingual preferred.
Knowledge of homelessness and/or lived experience a plus.
Possession of good driving record for past 5 years.
Ability to work independently and engage with all individuals of any gender, race or ethnic background in a calm and caring manner.
Valid California driver's license with access to a personal vehicle with current insurance that can be used for company business.
Fingerprinting/background check as required by the Department of Justice.
TB test required.
